OUTREACH
Our Outreach Team distributes food and necessities to individuals who are experiencing unsheltered homelessness. Whenever possible, we provide transportation to a shelter or other service connections and schedule a follow-up appointment with our Case Management Team to discuss longer-term needs and care.
Outreach Locations:
Newark, Irvington, Rahway, and other locations throughout Union and Essex County.​
CASE MANAGEMENT
Our Case Managers assess the health, social needs, and situation of those seeking housing to form a housing stabilization plan to navigate case management and support needed for each individual.
Depending on the case, this process can take from a week to more than a year.

Our Outreach & Engagement Team will be at the following locations on a weekly basis distributing brown bag lunches, clothing, and toiletry kits, while helping to introduce people to our Case Management program.

Street Outreach & Engagement
​
In Newark, our Outreach & Engagement Team meets clients where they are seven nights a week, 365 days a year. In Union County, our team conducts outreach five days a week. The team make shelter referrals, transport clients to shelters, and distribute brown bag lunches, clothing, and hygiene kits.

PROJECT CONNECT CASE MANAGEMENT
​
Picking up where the Outreach Specialists leave off, our Case Managers work with clients coming off the streets and into emergency housing to assess their health and social needs and identify barriers and strengths to obtain transitional and/or permanent housing. This process culminates in the co-creation of a housing stabilization plan.
​
The housing stabilization plan is a living document that guides the case management process and helps Case Managers assist clients to leverage strengths to overcome multiple barriers to obtaining housing. Depending on the complexity of an individual’s needs and the availability of community supports, the time spent in intensive case management may be as short as a week or longer than a year.
​
Our Case Management Team will help you maintain or obtain permanent housing by working with you to secure ID documents, benefits enrollments, emergency shelter placement, healthcare connections, housing voucher applications, and more.

Bridges Outreach is a one-stop-shop to help you help yourself. We’ll strategize with you to put together a plan aimed at housing, health, and independence, and connect you with resources.​Bridges provides you with Housing Navigation & Placement and Housing Sustainment & Skill Building.
